คุณ ชื่อ อะไร ครับ/คะ
khun chêu a-rai kráp/ká
What's your name?
คุณ[khun] is as "you"
ชื่อ[chêu] means "name"
อะไร[a-rai] equals "what"
Thai is different to English, if you look at the sentence "คุณชื่ออะไร", it is the question sentence as same as "What's your name?" in English.
We don't use the word "อะไร", which is the same meaning as "what", to lay at the front of the question sentence. On the other hand, we put it at the end of the sentence.
Hence, just use this form for making some more sentences similarly;
.... ชื่อ อะไร ครับ/คะ
.... chêu a-rai kráp/ká

For example:
